How do I go about to obtain my Medical Records?

St. Croix Orthopaedics is not allowed to release Health Information or Medical Records without the proper authorization. Please fully complete the Medical Records Release Form and return to the Health Information Team at the Administrative Offices in Stillwater.

Why do I have to sign a release for my medical records?

St. Croix Orthopaedics P.A. undertakes a legal and ethical responsibility to foster and preserve the privacy and confidentiality of patient information. Accordingly, St. Croix Orthopaedics will strictly adhere to obtaining prior consent of the patient before information is released, disclosed or made available for review except where a specific law or regulation or the internal administrative needs of the facility require or permit such access without patient consent.

Can you fax my records?

Yes. However, we do ask for written permission and the fax number before we will fax your records. Please be aware that not all fax machines are secure, so ultimately you need to be responsible for the privacy and confidentiality of your medical records.

Why is there a charge for films?

Original films are part of your permanent medical record and therefore are kept at St. Croix Orthopaedics, P.A. We duplicate the original film at your request and the cost that you pay covers only the expense of the film.

If I need to pick up this information, where do I go?

Your records can be picked up at the St. Croix Orthopeadics administrative offices at 1991 Northwestern Ave South in Stillwater. (See “location” tab for further directions.)

May I send someone to pick up my medical records or films?

You may send someone in your place to retrieve your records or films as long as you send a written permission slip for the person you are appointing to pick up your records or films. We do check driver’s license to ensure that your records are given to the proper person.

May my Mother or Father sign for my medical records?

Yes, but only if you are under 18 years of age.

How long does it take to receive my records?

  • Emergency requests involving immediate emergency care of patient: immediate processing
  • Priority requests pertaining to current care of patient: within one working day
  • Patient request for access to own record: within three working days of receiving request
  • Subpoenas & depositions: as required
  • All other requests: within five working days
